A female aged 90, bed bound, had a skin ulcer caused by double incontinence and inappropriate method of skin care, position change and pressure relief at home. Nu Pro electronic wound care consultation was offered as the patient lived in remote area with great difficulty in attending Nu Pro Nurse Centre. Her relatives cared about her very much but unfortunately they did not have the proper knowledge and skills in handling her.
Appropriate education, effective communication with care givers and proper monitoring of the wound at home were of great importance. In view of this, Nu Pro wound nurses had formulated an individualized care plan for the patient including proper technique and skill on position change and transfer, effective pressure relief, proper method of skin care and skill on handling the wound. The above were monitored promptly with daily photos received from the relatives. Progress of the wound was encouraging and quality of surrounding skin had improved significantly within 1 week. The caregiver was reminded to continue with the above in order to prevent recurrence of skin damage.
